Linking to a DiADeM Recipient

To be able to use the DiADeM app as intended, you will need to have a DiADeM Recipient to whom you will send your completed DiADeM Assessments. This will usually be the main email address of the patient's GP Surgery.

To be able to send assessments to a DiADeM Recipient there needs to be a link between you, and the way to form this link is for either the DiADEM Assessor (you) or the DiADeM Recipient (the relevant GP Surgery) to send a request to link via this DiADeM website.

If the GP Surgery has already registered with DiADeM you can search for them in the drop-down below - and then include a short personal message to them if you wish. When you complete this registration form and click the 'Register' button a request will be sent to DiADeM Recipient informing them of the request to link with them. They can choose between 'Accept' and 'Decline' buttons in the email they receive, and if they choose 'Accept' you will be able to select them as a DiADeM Recipient in the DiADeM app and send them Diadem Assessments. Note: the request to link email is only sent off once you have clicke the 'Register' button on this page and then verified your email address in an auto-generated email that will be sent to you straight after.

If the GP Surgery you want to link to has not yet registered as a DiADeM Recipient you can leave the linking to a DiADeM Recipient for now and continue to register. Once they're registered you can log in to your DiADeM account and set up a link request to a DiADeM Recipient from your profile page.
