Linking to DiADeM Assessors

To be able to use the DiADeM system as intended, you will need to have at least one DiADeM Assessor who will use the DiADeM app in a care home setting to carry out DiADeM assessments with one of your patients - and to then send you the completed DiADeM Assessments. Assessors will be GPs or other healthcare professionals who have a valid association with your healthcare facility.

To be able to receive assessments from a DiADeM Assessor there needs to be a link between you, and the way to form this link is for either the DiADeM Recipient (you) or the DiADeM Assessor (a healthcare professional with an association with your healthcare facility) to send a request to link to the other via this DiADeM website.

If the healthcare professional(s) you would like to link to have already registered with DiADeM, you can search for them and start the DiADeM request to link process from your DiADeM profile page (once you completed registration and have logged in to your account).

If the healthcare professionals you would like to link to haven't yet joined, we will shortly have a section in your DiADeM profile page where you can get the DiADeM website to send them invitations to register. Until then, you will need to contact them yourselves, and once registered, either they or you can start the linking process.
