DiADeM Documentation

There are various documents available here:

• The DiADeM Service User Agreement - this is the main Terms and Conditions agreement between Apperta and the DiADeM User (the clinician undertaking assessments and the DiADeM Recipient)
• The DiADeM Privacy Notice (for the Patient) - this is what should be made available to patients and if appropriate also to their next-of-kin and care staff.
• The DiADeM Privacy Notice (for the Clinician) - this is for both the clinician undertaking DiADeM assessments and for DiADeM Recipient
